Our HIPAA Privacy Work Group has implemented and will continue to develop and consistently monitor, review and update our Information Security Program, which includes:

1. Formal Privacy Policies & Procedures outlining the use and disclosure of PHI

2. An Information Security Program that addresses organizational practices, security of physical facilities and
electronic data, and staff training

3. Ensuring that all Third Party Providers have an equivalent program

As part of that Program, we are providing our third party vendors with a Business Associate Agreement and requesting that they enter into the Agreement with us.
